Letter of Inquiry
Organization Information:
- Name, address, and website of organization
- Mission statement
- Organization’s annual budget amount
- Federal tax ID number (or fiscal sponsor name and federal tax ID number)
Contact Information:
- Contact information for the Head of the Organization (i.e., Executive Director, CEO, President)
- Request Primary Contact person who will serve as the contact for any questions.
Project Description: (no more than two pages)
- Brief narrative description of the problem statement and project
Request Amount: Total amount of funding requested from the Lisa and Douglas Goldman Fund.
Please attach the following information:
IRS Designation Letter: Federal tax-exemption letter or a letter/agreement from your organization’s fiscal sponsor confirming the relationship.
Project Budget: A one-page itemized budget of revenue (indicated committed and anticipated) and expenses for the full project. Sample Project Budget
