Letter of Inquiry

Organization Information:

  • Name, address, and website of organization
  • Email and phone number of contact person
  • Mission statement
  • Organization’s annual budget amount
  • Federal tax ID number (or fiscal sponsor name and federal tax ID number)

Project Description: (no more than two pages)

  • Brief narrative description of the problem statement and project

Request Amount: Total amount of funding requested from the Lisa and Douglas Goldman Fund.

Please attach the following information:

IRS Designation Letter: Federal tax-exemption letter or a letter/agreement from your organization’s fiscal sponsor confirming the relationship.

Project Budget: A one-page itemized budget of revenue (indicated committed and anticipated) and expenses for the full project. Sample Project Budget
